making a gui

i am trying to make an application. I have written the code in vs and want to make the gui using qt. Is there a tutorial or step by step methodology to integrate them. Or is there a simple way to make the gui?
I just need a window which intakes 4-5 parameters, calculates some values and displays one single value.
