Call function on Address execute

I've been trying to find info relating to this and have had poor luck so decided it would be better to just ask.

I'd like to have a function called every time an address from an external process is executed, this is to replicate behavior of certain cheat disc master codes so that I can load their codelists directly to my app without converting anything to a different code type.

Any example code would greatly appreciated.
