Popup input box sent back to consle window

Working on a challange from my teacher to get a pop up dialog box to input information and send it back to the console window with a space between each. What is thee best way a bout going about this? I was thinking of somehow linking to a form but not sure how to start or if that is the right path.
Ohh and this is not for a grade or anything just busy work since i finish my labs to quick
